このプロファイルはDiagnosticReportリソースに対して、病理検査レポートのデータを送受信するための制約と拡張を定めたものである。

概要

本プロファイルは、病理診断レポートの情報を記述するためのDiagnosticReportリソースの日本国内における実装ガイドである。 病理検査は、組織や細胞を顕微鏡で観察し、疾患の診断を行う重要な検査であり、本プロファイルではその診断レポート全体を管理する。

背景および想定シナリオ

背景

日本の医療機関では、病理診断は以下のような流れで実施される：

  1. 臨床医が病理検査を依頼
  2. 検体（生検、手術材料、細胞診等）を病理部門に提出
  3. 病理部門で検体処理（固定、包埋、薄切、染色）
  4. 病理医による診断
  5. 病理診断レポートの作成と報告

このプロセスにおいて、病理診断レポートは最終的な診断結果を臨床医に伝える重要な文書である。

想定シナリオ

本プロファイルは以下のようなシナリオで利用されることを想定している：

シナリオ1：生検病理診断

  • 内視鏡検査で採取された胃生検組織の病理診断
  • 肉眼所見、顕微鏡所見、最終診断を含むレポート

シナリオ2：手術検体病理診断

  • 手術で摘出された臓器の詳細な病理診断
  • 複数のブロック・標本からの所見
  • 免疫染色結果を含む総合診断

シナリオ3：細胞診

  • 喀痰、尿、体腔液等の細胞診断
  • スクリーニング結果と最終診断

  • 病理診断レポート（組織診、細胞診、術中迅速診断を含む）
  • 肉眼所見、顕微鏡所見、診断結果の統合管理
  • 関連する画像（WSI、キー画像）への参照

  • 個々の観察結果の詳細（JP_Observation_Pathologyで管理）

status

  • レポートのステータスを表す
  • "preliminary"（作成中）、"final"（確定）、"amended"（修正）等を使用
  • 病理診断の確定状態を明確に示す

category

  • 病理レポートであることを示すため、LOINCコード"LP7839-6"（病理）を必ず指定
  • 複数のcategoryを指定可能だが、病理を示すcategoryは必須

subject

  • 患者情報への参照を必ず含める
  • JP_Patientプロファイルを参照

specimen

  • 病理検査では検体情報が必須
  • JP_Specimen_Pathologyプロファイルを参照

注意事項

データの整合性

  • specimen要素で参照する検体と、result要素で参照する観察結果の検体は一致している必要がある
  • 複数の検体がある場合、どの所見がどの検体に対応するか明確にする

ステータス管理

  • "preliminary"から"final"への変更は、診断が確定したことを意味する
  • "amended"は既に確定したレポートの修正版であることを示す
  • ステータスの変更履歴は別途管理することを推奨

画像との関連付け

  • WSI画像がある場合はimagingStudy要素で参照
  • キー画像はmedia要素で参照
  • 画像へのアクセス権限に注意
